Wish Team Resources

Here are all the forms you, as a Wish Team volunteer, need to take with your partner to complete a Wish interview with a child and family. We have a complete set of forms here, both in English and Spanish. Please fax, mail, or deliver the forms, filled out as completely as possible, to our offices as soon as possible upon completion of the interview.

2008 Wish-Team Training Schedule

For those who have attended the New-Volunteer Orientation and would like to become Wish-Team volunteers, the Wish-Team Training is held quarterly (subject to change). This training is 5 hours long and is required only for those volunteers who want to do wish granting and/or meet & greets. Invitations are e-mailed approximately 1 month beforehand to those who have attended the New-Volunteer Orientation.

• June 28 at Children's Hospital Los Angeles

• December 6 at Children's Hospital Los Angeles
